I am using cotton buds and chemically clear methyl-alcool to clean the filters after the treatment and for cleaning the spot size.

Charry,

Can you do 515 nm and 560 nm in the same session if the patient has pigment and vascularity?

Do you need eye shields or eye protection (will the eye protection get in the way of the treatment head?).

Can the patient just close their eyes if you treat outside the orbital rim?

It isn't necessary to use both the 515nm and the 560nm filter.If there is pigment and vascularity just use the 560nm filter with a little higher settings as i said before for the 515nm filter.You can use eye shields,if you want to,if the lessions are close to the orbital rim.

To any Profraction XC users,

I am considering the XC upgrade to my current Profile system. Have any users compared it to fractionated CO2? I am demoing SmartXide, and Mixto next week. Does anyone know of a head to head comparison? Do any users have any experience with these or Deep FX?

I have not yet used the Contour on the upper or lower eyelids, and am anxious to do so. Any suggestions on which eyeshield to go with? Also interested to hear about any protocols for anesthetic use on the eye for such. Depth/overlap percent/coag?
Also, any reports on the Profractional use in these areas?

BTW, went to the Aesthetic show and must say that I left fearing going too deep with the profractional. I have gone down to 800microns/5% for acne scars with local anesthetic and my patients are not complaining.

Lumenis has an excellent Webinar on the DeepFx from a few months ago. It is a roundtable discussion. Jeff Barson has posted a summary of that webinar on this blog. Check out the summary and then listen to the webinar. This information will give you an excellent background to compare and contrast the different CO2 and Erbium lasers available.

I think it is somewhere between $50,000 to $60,000 for both the Profractional and the Profractional XC.

I am not sure if you can purchase just the XC or if you are required to have the regular Profractional as well.

Hi. I am thinking about buying the BBLs. I already have the ActiveFX, The Fraxel and the Vantage 1064, so I don't need the other parts of the Sciton Platform.

I have a few questions:

1. How do you treat dark circles under the eyes with the BBLs? Is it effective if the problem is pigment or vasculature?

2. How do you do with pigment in asians? what filter do you use and what range of j/cm2, temp and pulse duration do you use?

3. Does the InfraRed (ST) filter really work for skin tightening?

Hi JEE

1)If the problem is pigment or vasculature you can use the 515nm filter or the 560nm filter and you would have a very good effect.
2)In asians patients you could use the 515nm filter but at lower settings starting from 9/8 J/cm2 and 20/25msec at the 1st session for skin types IV/V.
LH and dexter could help you more for asians pt.
2)The ST works very well for skin tightening.I have pre- and post- fotos after 1 year follow up.I could send you via email.

I just received my Sciton last month. I have a question about the ST. How many treatments are you doing on the face and neck and how often? Have you used it on the abdomen? If so, what is your protocol?

I am doing three treatments (ST) on the face and neck with 1 month apart one from the other.I have been using it on the abdomen with a package of 3-4 sessions (it depends on the patient's problem) with also 1 month interval between treatments.The results are impressive!!!I have been using up to 50J/cm2 and 6sec with cooling temp. at 13 or 14 d.C (post cooling 2sec) for patients with skin type near up to IV.I am doing 4 passes.For patients II-III i have used higher settings up to 55J/cm2 and 7sec with the same cooling temp.(13-14 d.C) and the same passes(4).For patients >IV i have been using a little lower settings (45J/cm2 and 6sec) with cooling temp. at 12-13 d.C.The results are very good and my patients are very happy with them.I have been also using a combination of BBL +ST for the treatment of stiae(red) and a combination of microdermabration +ST for the atrophic striae(lineae albicantes) with very good aesthetic results.Post me if you have any other questions.

Do you set your joules based on patient's discomfort? For example, 4 out of 10 pain?

Do you use the "big" head and the smaller "masks" or do you just use the "big" head?

Do you see immediate tightening or do you have to wait 3-6 months?

Do you have experience with the Titan or Thermage? If so, how does Skin Tightening with ST compare?

1)For Full face+ neck i am using about 130-140 pulses with the "big head".If you use the "small" head(15x15mm) the pulses are higher (with the 15x15mm head are almost the double).For the abdomen it depends on the surface.
2)The settings which are posted above are the top of which i am using (these are depend on the skin type AND the patient's discomfort).
3)I can see immediate tightening and the results are coming better and better during the time of 3-6 months.
4)I haven't any experience of Thermage or Titan.I purchased one year ago the ST for my BBL and now,as you saw on my patient's foto,the results are excellent with minimal downtime!!!So,why i have to search for something else?

I am a big time Sciton ST as well. I agree with Charry's parameter. I set my parameters at
Type III- IV skin:
First Pass: 55 J, 6 Sec Pulse Width, 1 Sec Post cooling, Cooling temp of 12 C on the first pass.
Second Pass: 50 J, 6 Sec Pulse Width, 1 Sec Post Cooling, Cooling temp of 12 Con the second Pss.
Third and fourth pass: 45 J, 6 sec pulse width, 1 sec post cooling, cooling temp of 12 C on the third and subsequent pass.

You must reach treatment end point of Mild Erythema (E1-E2) to get result. I tend to schedule 4 Sciton Skin Tyte treatment in one year spaced 3 months apart. The result is comparable to the result I get from a one time Thermage Thermacool treatment.

Sciton Hair Removal:

I have always used my Sciton BBL 695nm filter for hair removal (30-40ms, 15-17 J, cooling temp 12 C) with good result requiring 5-6 treatments per year for 85-90% hair removal success rate.

I also have the 1064 long pulse ND YAG laser in my profile module. I have used it with the scanner hand piece and got some patchy results. The scanner simply skips too many spots and has uneven coverage. Would any of you prefer 1064 ND YAG laser over the BBL for hair removal?

Sciton Hair Removal:

I have always using my BBL for hair removal for skin type up to IV.For patients >IV i am using my Nd YAG module of my Profile.
For type II-III i am using always the 640nm filter (up to 15-16 J/cm2 and down up to 15msec,cooling system at 11-12 C).With 5-6 treatments i have a 90% hair removal rate.
For type IV sometimes i am using the 640nm filter (ALWAYS >20msec and not higher of 13 J/cm2,cooling system at 10-11 C),but usually i prefer the 695nm filter (>20msec and fluence up to 16-17 J/cm2 ,cooling 11 C).

Thank you for posting your experiences with your Sciton. We've had ours now for about 9 months and I am still impressed with the platform.

I have been shying away from treating melasma with either BBL or profractional, however, I have heard anecdotal information that melasma patients have been responding well to profractional. Is this your experience as well?

I've been using combination of BBL then Profractional in same session with good results. Have you tried this as well? Do you do any other combinations? What have you found to be your best treatments for patients who can tolerate a long weekend of downtime?

I would like to see your pics on your ST patients. We are considering that module as well. My email: flightdoc@yahoo.com.
