Skin Type: Pigment & Complexion > Treating Hyperpigmentation caused by IPL

I have PIH from botox injections; not the botox serum but the injections themselves have caused 5 large brown marks at the injection sites. I have used Hydroquinone with and without Retin A. I've had Fraxel and IPL. Nothing has worked. Do you have any suggestions? 3 Drs have said they've never seen or heard of it before but they can't deny that the large brown spots (the size of the nail on your little finger) are exactly at the injection sites. It has been suggested to me that pigmentation caused by trauma will eventually fade. Is that true?

i had done fraxel for two times and ipl for more than 15 times in 3 years. my freckles were all cleared up until the last two ipl. i finally got two big brown spots next to my nose areas. the doc then try to peel them off with chemical peels and he made the marks worse. i found an esthetician and she has done cosmelan (without hq) for me once every 6 months. it's been improving a lot but she told me that the pih is going to be permanent because it has damaged my skin. i can cover the spots now with press powder and concealer. i really wanted to go back and stick with my freckles because they look more natural at the least. pls think twice before you make any decisions on all the laser treatments.
the pih has stuck with me for 22 months.

Hi,i have treated lot of Patients of PIH especially phototype V (Pakistani Patients) and have used different modalities.My initial protocol is as follow
- First line of action is 4% Hydroquinone
-If there is no response then i add Q switched Nd:YAG laser with lower fluence like 4 mm and 2.8 j/cm2 on weekly basis for 4 to 8 weeks
-Two years Earlier i have started entirley different protocol
-I am using Fractional laser treatment one session with very low fluence 9X9 spot size,400 mj energy and Long pulse Er:YAG laser with single pass and then after 8 days 4% Hydorquinone.It works very well in 80% cases.However for 20% cases i have to use Q Switched Nd:YAG laser
-For the last six months i am using another protocol for resistant melasma or PIH........ 5.8 j/cm2 ,3 mm spot size Q switched laser and it works very well.
-To conclude it is difficult to treat melasma and PIH and one can learn with his own experience and clinical judgement and no one can say that single modality is enough.

Hi, please help me. I got ipl treatment for laser hair removal on my legs, but my whole thigh was severerly burnt. Its been 6 months now and i have brown marks all over my thigh. What do you reccommend? I have tried LED and also cutera genesis treatments but they have not helped. The place where i got it done are now suggesting i try IPL again to treat the hyperpigmentation. I am just scared as i dnt want the brown marks to get worse. I am of indian origin of olive complexion. I am desperate for some expert opinions.

I treat pin due to ipl burn with my revlite laser.
I do once every two weeks. Expect 70-90% clearance after 6 treatments.
I use 8mm spot size with 2.2 setting PTP on. Slowly cook the pih for 4-5 minutes every time.
Find a clinic that has the revlite laser. This laser is amazing for PIH and pigments.

Thank u soo much Mono. The salon I am visiting (where the burns happened as the lady didnt apply conduction gel) have a Quantum IPL machine. I will inquire about the RevLite. For the past few months I was getting Cutera Genesis done, but last week they started with the Quantam IPL to treat the marks. The marks have become slighter darker now so I am getting worried, but I was told this is expected for the first week and then it lightens? I hope this machine is ok to use? the last thing i want is for the marks to get darker.

Is the Quantam IPL as good as the RevLite or do you reccommend I see another place with the RevLite machine?

Really appreciate the assistance.

Hi all,
I had laser hair removal 9 years ago with a LightSheer laser. I am a Fitzpatrick IV-V, in other words I am dark-skinned (brown, not dark brown or black). The laser used for my treatment back then was not appropriate for my skin type and I was left with a small patch of hyperpigmentation that is still present on my face 9 years later.

Could the Obagi product line help remove this hyperpigmentation? Would a chemical peel? What are my options if any? Thank you.

I had a bleach burn 6 years ago and it has left me with PIH at the sides of my mouth, Is there any way I can get it treated? I would never go for the laser procedures since my skin is very sensitive. It looks like a dermal PIH and if I start on hydroquinone 4%, will I have to use it forever? I have used it twice and it has not helped me a lot. Any advice? Thank you.
