Physician to Physician Discussions > Need upper eyelid advice

I have a 45-year old client who probably could use an upper lid blepharoplasty but can't afford the surgical fee which is uncovered by her insurance. She asks how much tightening could she get from laser. I have a new Sciton 1064 & BBL / Skin-Tyte, and am exploring the limits of usage. I've done lower lids with some early success, but haven't done more than micropeels and 50um peels on upper lids so far. Can I use the same settings on uppers as lower lids? Settings for the last lower lids I did were: 1st pass 50 um without coag, 50% sport overlap; 2nd pass 40 um, 25% coag.

I appreciate hearing about others' experience using Sciton for upper lids

Upper eyelid skin is so redundant that a LASER is going to have minimal if any effect on improving her appearance. I am sure that by looking at her you are able to appreciate what she is after aesthetically and a LASER is not going to achieve her goal-an upper bleph can be done for around $1500 if she searches around.

Ben,
I have been doing 80microns ablation with 50% coag, 2 passes with density of 35-50% with my Sciton. The down time is at least 4-5 days with erythema for a week or so more. You can see the skin contract with the treatment. Make sure that the lower lid doesn't have much laxity, a good "snap" and the patient understands the healing timeframe. I personally, don't think the profractional works well around the eyes. You need heat and contraction to see results. Good luck. Steve

Thanks for the advice -- I did something close to what Steve recommended. 1 week result -- she's quite happy. So far, so good.
