PDT with ALA seems to be living up to the billing its been getting. The delivery method for the ALA has been of some debate. Some studies have suggested that the "best" (meaning most effacacy within clinical time constraints) method of ALA deliver before a PDT treatment is microdermabrasion followed by ALA applicattion for 15 minutes or so. Some other physicians are using ultrasound, longer length of application, or nothing. Has anyone seen results that would favor a specific system?
I'm using sonophorisis. I've heard that this has the greatest effacacy in driving the ALA through the dermis. I'm also leaving the ALA on for approx 30min but would like to have something faster. Anyone?

I am just beginning to use ALA in conjunction with IPL. Do you ever use topical anesthetic prior to the IPL? If so, would you apply it after the ALA has been on the desired amount of time eg. 30 minutes and then wait another 20 minutes or so for the topical to take effect?
I have cleansed the ALA after desired time and then applied topical anesthetic for 15 minutes prior to IPL
At the Annual AAD, Dr. Jason Lupton suggested that we use acetone prior to ALA application and/or microdermabrasion. I'm interested on feedback as to the length of application. 30 minutes, 1 hour, 3 hours? I know there have been studies...anyone with experience with specific incubation times? Also, is everyone using IPL or is anyone using blue light (Dusa, Clearlight, etc)?

Contact DUSA directly. They will set up training for you. Your MD hopefully should be well versed on this and have this information -- clinical trails and papers. This is pretty basic stuff. Why are we having this discussion ?
