What are available treatment options in the market for rosacea?

We've been treating our pts w/ a gentle cleanser and zinc sunscreen (and salicylic toner). But make sure to test your pt's skin first before proceeding with this treatment. We've also been recommending these to our pts w/ acne. We've been getting beautiful results.

I've had excellent results using the Yag ...It is a better alternative to standard oral antibiotics regimen, gels and other irritating topicals.

Hi NileAli,

What protocol/settings do you use for rosacea treatment? Thanks much.

I am a LE who has had rosacea for over ten years. I have tried the antibiotics to gels to creams prescribed by a derm with unremarkable improvement. However, I have been getting IPLs to treat my rosacea and I can tell you that it works.

IPL for rosacea? i've been treating my patients with IPL which only controls the redness, but this ttt doesn;t really eliminate rosacea. i am interested to know what's your skin care regimen at home?

We've been using Revitalight's RED light. Patients come in every 7 days for 1.5 months and they're pleased with the results.

The Dermasound Elite by GlyMed Plus is the best device for Rosacea skin. It actually has been clinically proven to reduce the Rosacea skin condition. My clients are amazed and love the results we achieve. There is no down time and inflammation disappears right before your eyes!

Dermalogica's Ultra Calming system will: reduce inflammation, reduce redness, soothe irritation, reinforce barrier lipids, stimulate synthesis of ceramides, add surface lipids and hydrate the skin.

If you don't know for sure what causes Rosacea...how can it be treated? Are you treating the symptoms?

Cutera's 'Laser Genesis' is providing successful treatment parameters for diffused redness often associated with Rosacea.

1064 ND Yag treats superficial as well as deep facial telangiectasia. It also treats redness, small bumps under the skin and breakouts associated with acne rosacea.

My gold standard (that works 90% of the time) for rosacea is simply tetracycline 500mg daily x 10 days to get rosacea under control, then 500 mg on Monday and Thursday only for maintenance. Works better than any other regimen and has the bonus of being cheap! Can substitute doxy but doesn't work as well as TCN.

Pevonia RS2 is very good product with proven results ,client has to use it 3-4 weeks to get results .

To remove visible redness, we use the 940nm diode laser.

SOOTHE from Rodan+Fields is patent pending and has been featured at the American Academy of Dermatology meeting. Works wonders.

There is no such thing as a 940nm diode laser. This is an IPL attempting to imitate the function of a 1064nm Yag laser- the most effective laser for rosacea.

Remember- IPL is only an imitation of laser, and can never be physically equivalent.

Try to do some research and find masks/ skin care products containing aloe and/or azulene; azulene works wonders for sensitive skin.

Besides medication options, as far as for lasers, Coolgilde vantage is ok, Fotona 1064 300mu second is good, the best is the new coolglide excel V long pulse 532, it is the best so for for my experience. It is very good for diffuse reddness, reddness for acne marks and port wine stain, no purpura.

I don't own the coolglide excel V because it is way too expensive for just one condition. I have tested the machine, excellent for diffuse reddness and no downtime, and if you can spare US 100000 for such a toy, good effective laser. I want to own it someday.

IPL is very un-predictable, usually it makes the condition worse especially if the rosacea is on a flare.

There is a 940nm diode laser. All you have to do is google it. If you're going to try and pass yourself off as an expert at least do a little research and fake it.

Also, please stop with the "IPL is only an imitation. . . " nonsense. My guess is that you have never actually treated a patient and if you have your experience is very limited.

Dear Anonymous,

LASER is Light Amplification through Stimulated Emission of Radiation.

There are several major types of lasers:

Solid state lasers, which use crystals as the medium for stimulated emission of radiation. That is, a change in energy state causes the medium to release a photon that is always at a wavelength determined by the material itself. The energy state of a Neodymium doped Yittrium Alluminum Garnate crystal (Nd: YAG) is stimulated by a flash lamp, and the photons released are at a 1064nm wavelength. Alexandrite, Ruby, Holmium Yag, Erbium Yag, etc. are other solid state lasers.

DPSS, or diode pumped solid state lasers are smaller versions of the above mentioned lasers, but use a powerful diode to stimulate emission from the lasing medium- diodes are NOT the lasing medium, and these are not referred to as "diode lasers." Most eye lasers are DPSS.

Pulsed dye lasers use a radioactive dye as the lasing medium. Candela VBeam, for example, is a pulsed dye laser.

Excimer, or excited dimer, are lasers that use a difference in energy state using a gas mixture and a chemical reaction. They are used mostly in heart surgeries where minimal tissue damage is desired.

Then you have the gas lasers- Argon, CO2, etc. These are usually either DC or RF excited, and the gases release a photon within a specific range.

Diode lasers, or what are referred to as diode lasers, are not lasers. While there are diodes that can be considered lasers in that they are designed to form a difference in potential between two coated mediums, and allowing for spontaneous emission of radiation, which causes an oscillation, and in turn, stimulated emission of radiation- what you are referring to does no such thing. What you are referring to is an uncollimated row of light emitting diodes (LED's) with a coating that blocks broadband light, and allows a short band of light to pass through, and this light is then focused through a crystal, and applied to the skin much like an IPL.

I have worked with Rosacea patients and hands down lasers( ipl and diode ) are the best for keeping this condition at bay. Topicals help to further these results. Most patients have been on numerous topicals from doctors with no luck. A good skin care system, laser treatments, and sunscreen has worked well for me. I really like skin medica redness relief, Neocutis peche and opc gel from theraderm. There are some great options, we just need to educate clients with the right info.

Rodan and Fields Soothe regimen is amazing on rosacea, eczema, psoriasis and sensitive skin. I have psoriasis really bad on 60% of my body so right now I only use it on my legs. It really calms the itching, scaling patches.
