The only effective option is surgery to revise the pockets, either through a peri-areolar or inframammary incision. If you already have one of those 2 incisions, you won't have any new scar. If not, the peri-areolar incision generally heals near-invisibly, and provides excellent access to close the parts of the pocket that are allowing your implants to ride too closely together.
Shouldn't she be having this conversation with her plastic surgeon?
Smaller submuscular implants. Even if her skin can be stretched her frame may remain too small for the implants.

Patient had breast implants done about 1 year ago. She really had very little breast tissue at all. Implants are nearly touching in the middle with minimal skin interposed and she is unhappy about how it looks. Skin is great at stretching in most areas but I am looking for ideas on nonsurgical options to get her implants to separate and look more like "normal" breasts. She is very self conscious about this and while she knows a pregnancy would likely help, she doesn't want a baby right now. She is open to suggestions but does not want another surgical scar.
