hi everybody.

I have been reading for a long time posts about laser and ipl devices , read clinical studies , and yet i can not understand if over all the efficiency of lasers (alex mostly) is better then the ipl (harmony , starlux , prowave) or with both one can get the same results. i would love to know the diffrences according to your expirience when treating diffrent parts of the body (mostly upper parts - i am a male) and when treating diffrent hair types (black course and black fine hairs)

Setting for skin type 4-5 clients. ND:YAG laser. Having trouble clearing side burns for east indian ladies. Hair gets finer and longer? Any treatment parameter suggestions. Upper lip, chin clear very well

We have been using IPL and 1064 the same day with a little better results but be very cautious with the east Indian ladies when using the IPL.

any information related to skin types I-III ?

If you are going to do only hair removal on fitz I-III then go with an alex. If you are going to do more than hair removal then use a multiplatform system.

We have a Nd:YAG laser for hair removal and have learned that we get the best results when when we "push the envelope". It is not uncommon for us to use 10cm spot size,75-80j/cm2, 10-15ms pulse width on the underarms (not usually a tanned area). We always perform a test spot. Very important to precool (and post) the skin and make certain the patient has recently shaved the area.

Interesting idea about using IPL and 1064 at the same visit. Just curious what IPL wavelength are you using. I would like to give this shot on our really tough patients. Thanks in advance for the feedback.

I am seeing better results combining Sciton 1064 and BBL at same visit. I have an article by Dr.Michael H. Gold in Dermatology Times, Nov 2006 that supports this theory.

I don't like IPl for hair removal. In response to the question about east indian ladies and the hair just getting finer and longer-I have seen this! Sooo scary and almost impossible to reverse. Sometimes they even get MORE hair. happens to certain hispanics as well, mostly dark 4s and light 5s. I have found that if you do not treat these types aggressively enough from the start this is very likely to happen and results in some very unhappy campers. I would suggest doing what we have done in our office and nominate one person in the office to treat all the dark skin patients. Leave it to one person; (a good one that won't get complacent and burn the patients, but brave enough to try it) Use the 755 alex as much as you can, YAG as a last choice. There seems to be a very fine edge here between what makes and effective tx and what will burn, quite a learning curve.

I think purespa may be right about the IPl in use for darker skinnned individuals and hair removal. In order to use IPL on ethnic skin you must basically undertreat. IPL has to braod of spectrum of light. Therefore, you can not push the energy levels and you must increase your pulse width. This is likely to make it so that you do not get enough energy into the shaft of the hair follicle. We never use IPL on our darker clients except in conjunction with 1064 on the same day.

I would also agree for the shops with multiple people doing LHR to allow one individual to treat all ethnic patients. It makes since to have one person really good and comfortable with it.

I am not sure why purespa feels that the Alex laser is first choice over the "yag". The 1064 is really the best for ethnic skin with a higher safety profile. I would agree with the statement for fitz 1-3.

I've been using Aesthera's PPx/Isolaz for HR (sides by the ears) on the Asian/Indian types and getting positive results. It seems to work well on finer hair. I use the 580nm tip and always start low. As I gain more understanding of their skin response I do "up" the power. BTW, I do encourage them to buy a "package" to keep them motivated. After 6 tx I treat them @ no charge since it only takes me about 15 minutes. I want happy patients!!!
Using the Aesthera PPx/Isolaz I have gotten really good results.
I'm not so sure about pigment; I'm tending to start them off with a peel. Just starting using the Vi-Peel. I always want to accomplish the most for the least amount of $$. Pts do like the approach.

I have been treating a caucasion male (24) at our clinic for mild rosacea with the Lumenis One and yesterday he came into the clinic with three pock marks that he says spontaneously appeared within a week after the last treatment I gave him. There is no sign of any burns or erythema and I am literally scratching my head trying to figure out what is going on here. He is threatening legal action.

We took at look the before photo and the pock marks were not visible. I am thinking this may be some kind of rare immune response that may or may not be related to the IPL? I have not seen anything like this in the year and a half that I have been using IPL.

I need to know which is more effective, the alex. or the isolaz? which one yields better results? I wish dr. lanzer would be cheaper and more competitive considering the facts about the machinery and considering they cannot offer any real guarantees. We pay so much all for trial and error? Are we vulnerable to the medical profession?

I am not sure that I would want to go to a person just because they are cheaper. A lot of times you get what you pay for.
