Bodysculpting & Liposuction > Mesotherapy vs. Liposuction

I've heard various opinions on Mesotherapy. Are there any docs here that would share their experiences. I'd like to keep an open mind although the predominant opinion I've run into is that it's unproven. Thanks.

We've been using meso for more than a year and I couldn't be happier. Our patient satisfaction rate is as high or higher than liposuction and I don't have to charge as much. We've also found that the effect is usually more even, especially in smaller treatment areas.
I've been performing mesotherapy on myself and family members. It works. It works so well that I'm opening my own medical spa to offer the services as well as other non-surgical procedures. The problem out there is people who are not medical professionals doing meso. Where I live it seems at least twice a month someone unlicensed is getting busted. This hurts us all leading to criticism and skepticisim about the treatment and it's effectiveness.
Meso can treat so many things from acne to hair loss and gynecomastia. In my opinion it's fantastic. Check out all the procedures I offer on my website for more on what you can do with this great treatment.
Are there any pre/post photos of meso? just how much fat can you work with? are there any patient thoughts about the treatments?

I have been performing mesotherapy and lipodissolve for the last three years and have had great success. It works well and patients are happy. I have performed mesotherapy on myself and am quite pleased with the results.

Our spa offers mesotherapy with fabulous results. I have had it done on myself and love it. The only problem we have ever had was patients that could not handle the pain following the treatment. For me it is usually just a few hours, but some patients report pain and inflammation lasting up to 2 days. If they stick with it htey are always glad they did.
Check out Board American Society of Plastic Surgery ---- on Mesotherapy. What do they say?

Patients can/may legally sue you for treatments unproven and NOT recognized by American Board of Plastic Surgery. It doesn't matter if it works or not. Physicians beware. It will not hold up in court. Ask your malpractice agency. Mesotherapy at your own risk.

Mesotherapy not proven as a safe alternative to liposuction
Main Category: Cosmetic Medicine / Plastic Surgery News
Article Date: 19 Apr 2005 - 0:00am (PDT)

The allure of shedding unwanted pockets of fat with a series of simple injections, known as mesotherapy, sounds too good to be true - and it just might be. According to an ASPS Device & Technique Assessment (DATA) Committee report published in the April 15 issue of Plastic and Reconstructive Surgery® (PRS), the official medical journal of the American Society of Plastic Surgeons (ASPS), patients should be wary of mesotherapy until the safety and effectiveness of the procedure are confirmed.

"The promise of a non-surgical, permanent method for fat removal and body contouring is obviously very appealing, but mesotherapy is not proven to be the miracle cure to a thinner you," said Alan Matarasso, MD, study co-author. "The problem with mesotherapy is the whole technique is shrouded in mystery. Liposuction remains the only proven method to safely and permanently remove fat."

Touted as a non-surgical alternative to liposuction, mesotherapy involves injecting medications and plant extracts into layers of fat and connective tissue under the skin. The injected ingredients may include agents that are used to open blood vessels, non-steroidal anti-inflammatory medications, enzymes, nutrients, antibiotics and hormones. Mesotherapy may be used in conjunction with dietary modification, hormone replacement therapy, exercise and nutritional supplements. No drug is approved by the U.S. Food and Drug Administration for use in mesotherapy.

Although the practice of mesotherapy has been around for decades in Europe, it has only recently been introduced in the United States. There are no published scientific studies demonstrating if the effects are permanent, why or if certain ingredients work, or how safe mesotherapy is for patients. In addition, there is no standardization in mesotherapy. The types of drugs used vary from physician to physician as does the quantity and frequency of injections.

Mesotherapy may cost $1,000 to $1,500 per treatment with three to six treatments required. This can prove costly for patients who may not receive the desired results.

"Plastic surgeons are constantly researching better methods to help patients achieve their desired look; however, patients' health and safety is always foremost on our minds," said Dr. Matarasso. "There is no information on what happens to fatty acids once they leave the targeted area or how the various ingredients affect the body's organs and other tissues. There is simply too much we do not know about mesotherapy to say it is unquestionably safe for patients."

Currently, liposuction is the only method that has been proven to safely and effectively remove fat. According to ASPS statistics, liposuction was the most popular cosmetic surgical procedure in 2004, with more than 325,000 procedures performed.

"Without data to support claims that mesotherapy works, the procedure appears to be a mystic type of therapy," said Susan Kaweski, MD, ASPS DATA Committee chair. "The DATA Committee has not seen any long-term studies that would prove mesotherapy actually removes fat from the body. We need to know a lot more about this procedure before plastic surgeons can endorse mesotherapy and recommend it to patients."

The American Society of Plastic Surgeons is the largest organization of board-certified plastic surgeons in the world. With more than 5,000 members, the society is recognized as a leading authority and information source on cosmetic and reconstructive plastic surgery. ASPS comprises 94 percent of all board-certified plastic surgeons in the United States. Founded in 1931, the society represents physicians certified by The American Board of Plastic Surgery or The Royal College of Physicians and Surgeons of Canada. http://www.plasticsurgery.org.

I have done meso on myself on my abdominal fat. Was excited to learn it since it seemed so risk free. My first complaint was that there was too much down time- I had to wait at least 1 week b4 I could run again since it pulled so much. I had a lot of bruising as well, and now 3 mo out I still have discoloration from the bruising or possibly from the injections themselves. Has anyone else experienced this?

ED,
I'm hoping your a doc and not an hair dresser: http://www.medicalspamd.com/the-blog/2006/7/7/lipodisolve-mesotherapy-horror-posts.html

I find it rather disingenuous of the ASPS to completely discount the use of mesotherapy in Europe and Latin America. While I'd be the first to say that mesotherapy isn't for everyone and it's not going to work on people that are 30+ lbs overweight and a lot of others. It does have its place as a minimally invasive alternative for those with a few inches or a little fat to lose.

I guess that "With more than 5,000 members, the society is recognized as a leading authority and information source on cosmetic and reconstructive plastic surgery." the ASPS knows little of non-surgical procedures.

These turf battles do little to help the patients.

Ha Ha! Yes, I am a doctor, who was taught how to do this from another doctor. I did it on myself before I would ever do it on anyone else b/c I want to deliver what I promise and make sure it is relatively risk free. I am glad I did! I still have not performed this procedure on anyone else since I do not like the discoloration and do not see any results after 3 treatments.

Ed,

I have had two pts out of hundreds actually get that discoloration your talking about. It appears after the bruises have almost faded away. One black lady I've followed still has the discolorations from 9 months ago. Both the pts it happened to were either black or hispanic of dark skin. I don't know if it will ever fade or what has caused it. The black pt got it after the first session. The hispanic after her 8th session. Go figure.

Personally I love it and see results on myself after each session. I have never had a problem using it on caucasion skin.

Ed: if you couple the meso w/ intense diet and excercise it works great!

Hi all,

Mesotherapy is being used in Europe and other countries fro many years now with good results. Even if ASAP demonizes as not well studied etc, it does not mean that it is not effective or dangerous to use.
I will just refer to the fact that since it is a non invasive technique, it means that MDs, RNs etc can offer it whereas liposuction can only be berformed by a licensed MD. Money is always involved in these things even though nobody admits to it......
Can anyone share with the rest of us the active ingredients that were injected to get results with meso?
Stav

The only ingredient that have shown benefit for fat is Deoxycholate. Lipodissolve is a combination of phosphatidylcholine and deoxycholate. It ws originally thought the the PPC was doing the job but that did not turn out to be true. They were using the DC to keep the PPC in suspension and now they have found that it is the DC causing lipolysis.

The big issue I have is that these meds are NOT FDA approved. About 2 years ago the FDA put out a statement that the medications have NEVER been FDA approved for any use in the US. They have been used in other countries. Currently there is one company supporting research on PPC/DC and trying to get FDA approval. There is a research project under way and should be done in the next 6 months or so unless the study has been held up.

It concerns me that there are comments floating around that Mesotherapy is "safe" and "risk free." I guess I must be the minority out here that believes Mesotherapy and Lipodissolve should be removed from market. I had Meso performed about 6 years ago by an RN who specialized in Mesotherapy, which I suppose means that she took a weekend long class, then ordered a meso gun and mixed together a cocktail of lord only know what as a solution. Right when she started injecting me, I felt like something was very wrong as I suddenly was feeling intense pain. My injector told me to just hold on and that she was just about done. When I was driving home, I felt sharp pains, then two hours later my stomach started swelling to the eventual size of a basketball. After a sleepless night, I awoke to a black and blue, swollen abdomen. Just to give you an idea of the pain I experienced, when I untimately had liposuction four years later, it was a piece of cake compared to the pain of one session of Meso. When I called the nurse, I was told that this effect was completely normal and it would go away. Eventually, she quit returning my frantic calls. As my husband is a Surgeon, I felt like I should have known better for doing something so risky and putting my life in danger when I had small children. This lady would only refund me a portion of what I had paid for the remaining sessions. I was so angry with this woman that I filed a complaint with the state board. Mesotherapy was eventually banned in Kansas (where I and many others filled our complaints) but I heard the ban was lifted.

It's no suprise to me that the ASPS does not endorse Mesotherapy. Don't they, as well as all physicians take an oath to "first do no harm" to the patient?? Why on earth would a surgeon want to give a patient risky injection which might not work and could lead to 9 months of discoloration and a lawsuit? Not to mention from what I have read, scientists and doctors do not know where the dissolved fat goes after injections. One theory is that it could lead to a fatty liver.

Also, just because they are doing it in Latin America and Europe for goodness sakes, dosen't mean it is safe. Geez, when I was in France a few months ago, they were selling those pills that we had here in the states in 80's to give you a tan. The FDA removed those pills from the market decades ago because they were causing liver failure, but I'm sure in France they have used them for many years.

Hello,
I've heard a few stories about Mesotherapy and ill effects. First it has been my opinion as well as the most prominent pratitioner's of Mesotherapy in France that this is a "Medical" treatment. Ultimately it is up to the individual licensing agency to decide who can do whatever treatment. Saying that, there is no substitute for good training. I offer Mesotherapy to my patient's as well as VASER liposuction and Smartlipo. Each has advantages and disadvantages and have had excellent results with either. The fact is that more and more Plastic Surgeons are offering Mesotherapy in their practice as it works. As for FDA approval, both Deoxycholate and Phosphatidylcholine are used in FDA approved medications. They will never be approved for treating fat as there is no financial incentive for a pharmaceutical compnay to invest hundreds of millions of dollars for medication which is readily available through a compounding pharmacy.
Hope this helps.
Dr. Pitera

Dr. Pitera,
I almost choked on my after work glass of wine when I saw you actually posted this nonsense:

“As for FDA approval, both Deoxycholate and Phosphatidylcholine are used in FDA approved medications. They will never be approved for treating fat as there is no financial incentive for a pharmaceutical company to invest hundreds of millions of dollars for medication which is readily available through a compounding pharmacy.”

Are you kidding me? If they could get FDA approval, do you know how much $$$ these companies could make? There is nothing BUT financial incentive for these companies to get the FDA’s blessing.

Furthermore your statement that “both Deoxycholate and Phosphatidylcholine are used in FDA approved medications” is just incredibly misleading.

Phosphatidylcholine can be found in foods and is currently a dietary supplement in the United States, taken in oral form only. Although phosphatidylcholine is one of several ingredients used in certain FDA-approved intravenous drugs, it is NOT FDA-approved in combination with deoxycholate or for use in aesthetic intravenous purposes.

When I worked for a MedSpa that offered mesotherapy several years ago, I was horrified to find that there was a “secret” formula that was being injected, as other ingredients such as vitamins, minerals, and herbal extracts were added to the mixture.

I quit after a few weeks because not only is the FDA unaware of any credible scientific evidence that supports the effectiveness of mesotherapy, the agency recently issued an import alert against the zipmed.net and mesoone.com to prevent the importation and distribution of unapproved lipodissolve drug products into the USA.

I also challenge your statement that” The fact is that more and more Plastic Surgeons are offering Mesotherapy in their practice”

In my area, all the practices that were offering it dropped it in favor of Smartlipo and Zerona. Where are you getting that “fact” from? What documentation do you have to support that claim? Are you talking about practices in the USA or France?

I have heard wonderful anecdotal evidence that mesotherapy is effective in Europe and particularly in France, but until there can be a way to standardize treatments and there is complete transparency as to the actual ingredients, I would never add to it to our practices.

The ASPS was working on an FDA approved study to look at the efficacy of PPC/DC. They started the process a couple of years ago. Has anyone seen anything with that study such as preliminary results or possibly the study itself? The study was being sanctioned by the ASPS.
