Hi Midwest,
I started using it very recently for melasma, PIH from fraxel and other harder to treat pigment that IPL can't pick-up. I am hopeful and have heard many good things about it so time will tell. I am "cheap" in this town but my patients will still complain if they don't get fantastic results!

I have noticed some very resistant PIH from Reliant Fraxel also. Was it in your treatment protocal to keep the patients on a hydro-q throughout their Fraxel treatments? That seemed to be the trump card for many of our past patients.

I did pre/post care for these patients and follow-up. I can't wait to see your results with the Cosmelan. Are you going through a certain distributor that you found to be credible? Do you notice that patients almost go through an 'Efudex-ish' red and peeling phase? Is your charge for in clinic application and product around $800? I'm really curious about this product!

I charge $850 for the pack which includes the application, the C2 and Hydra-K creme. I tell them they have a week of reddness and peeling. The C2 can also give them a little erythema so sometimes we back down and use a little 1/2% hydrocortisone for a week.

I did not have my original fraxel patients on HQ until after it showed up and am still dealing with it. I will not use fraxel for melasma-it makes it worse and it still comes back where it was before even in the best cases (what a joke).

BTW-they changed the Cosmelan formula so it is not as "irritating" that first week or so they say. I have had 2 people who thought it was not as good as it used to be!!! Not sure what to make of that...
I am going through Mesoesthetic. Do you have other suppliers???

Our MD has done 2 cosmelan peels on darker skin with great results. We too have it listed for $850 and it scares people away. We have run it as a special for $600 with no luck. The 2 week downtime contributes to the scare.

I am not a doctor but have struggled with melasma for years. Cosmelan 2 was the only thing that worked and I have used it for 2 or 3 years. I first used it for 2 weeks as suggested, then daily, then once a week or if I saw a return of the dark spotches. I just purchased the "new" formula. It is not the same at all. Different texture, color and the results not nearly as good. It is really disappointing. Here in Canada, I paid $260 CAD which is completely unreasonable based on the European price. One place quoted me $170 then charged me $260 and I realized that $170 was probably the suggested price. I have purchased it in California for $175. Other than that, this was a great product. I had tried a 4% hydroquinine with Retin A, tri luma, peels, etc. Only Cosmelan 2 worked and I never did the mask. There are a few forums where users are sharing information about their results with Cosmelan. You might check it out.
....this is frustrating thing to deal with....

We have been using Cosmelan in our Michigan office now for almost a year. We have had much success, however, you need to make sure to warn your clients that any type of unprotected sun exposure will cause the pigment to return. We charge $700, and include a full size Cosmelan 2 and Hydra Vital K to make sure they use the product for 6 months. So, they are actually paying for the inital application of the mask, the products that come with the kit and two additional products. We have been very successful in selling the treatment at this price.

Cosmelan and Jessners peel appear to work great for epidermal melasma. Dermal melasma is the bugger;the Q-switched ND:YAG should work great as a long term fix. Just got mine : I'll let you know.

LH: the one to get (if you can find it used is the Conbio (medlite) C6. If you're not going to do a high volume the Medlite 4 has all 4 filters to get Black/Blue, red/brown, sky blue, and green. The C6 is the newer version and a little faster. I have the equivalent of a Medlite II that does 1064 (black/blue) and freq doubled 532( red/ brown). But we have a couple "bike-weeks" in Myrtle Beach (w/ the people that regret their tattoos) and I'm thinking of upgrading to the medlite4 ($20-30K used) or C6 if I have the volume. I neat site out west is Dr Tattoff (drtattoff.com) . But for melasma the 532 /1064 should be OK

Thanks, You reinforced what I already had been thinking. I was recently looking and saw a couple of used C6's for sale. I have not seen a C4 though. Doesn't Asclepion (sp?) also have one? Just not sure if it is a good choice. I am definitely leaning for the Medlite.

You had a post a couple months ago about smart lipo (cynosure) being underpowered but you were trying it out as well as a competitor. Do you have any followup information on this. I was not able to find any posts. I've heard good things lately about the smart lipo, wanted your opinion.

They apparently increase the power from 6 to 10 watts. So that sucks if you were an initial buyer and having to pay for an upgrade less than 6 months after purchase. Cooltouch CoolLipo has one also. I am waiting for the Sciton profile to come out. It will have 15watts of power and you are also able to use it for endovenous work.

The Smartlipo and CoolLipo are 1064nm and the Prolipo is 1320nm. I am hearing that the 1320nm is the better choice for fat destruction but the jury is still out. I have not purchased as I am waiting for more information as to how well they work etc. Many are using in together with traditional lipo.

Sorry, if you though I had started using. I do have a friend using the SmartLipo for about 3 months. I will try to call and see what he thinks.

I am definitely wanting a "smartlipo" or "coollipo" or the Sciton machine. I heard that you can buy the smart lipo in Europe for $6000, but because Cynosure has the patent, you have to pay them the extra amount. The 6 W is 95,000, the 10 watt is 125,000. The 10 watt works 50% faster. To do a complete abdomen(upper & lower) will take you about 4 hours with the 6 Watt. It is recommended to do only 3 areas, so total amount of time with the 6 W may take you up to 6 hours. Does anyone know more about the Scition machine and its projected cost?

I would wait a few months. Sciton's will be released at the ASDS meeting in a few weeks. I believe they are FDA approved. I am not sure about price but I think it is going to come in under the SmartLipo.

I was talking to a Cynosure rep at a conference and he mentioned something about a patent infringement lawsuit against Sciton. It had something to do witht he way the energy is delivered.

I am waiting a couple of months to see what happens. I already own some Sciton equipment and I like the 3 year warranty on them. Hopefully Sciton will give the 3 years on the ProLipo as well.

Sciton's Prolipo is a 1064 and is FDA approved. As you know, Smartlipo is also a 1064 and the pricing being quoted above is accurate on the S/L.

CoolLipo is the only FDA approved 1320 and has both a dedicated lipo unit and a combo lipo/endovenous unit. The combo lists for 100K and the solo for 90K. But the big story is the 1320 over 1064. Two prominent docs are presenting a study soon that shows 1320 melts fat at over twice the rate of 1064 and most importantly, with less heat, i.e. safer.

Since the patent was mentioned, all we have is Cynosure trying to scare the docs. If you've been around the laser business for long, you know there are always patent issues flying about, but the companies don't drag the doctors into the fray. If you ever have a chance to see their patent, it's interesting. Their patent revolves around the tip of the fiber being placed adjacent to the end of the cannula. If you fired the laser in this manner, all you would have is a red hot piece of metal moving through the tissue. What they teach in courses and to buyers is to extend the fiber tip out 2-4mm from the end of the cannula. The patent issue is just a sleezy tactic from a company with the wrong technology, that's actually a dental laser that you could buy for $40K a couple of years ago.

Long story short, get a hold of CoolTouch or wait for other 1320's to hit the market.

This thread has gotten off the track of Cosmelan but I'm here to reel it back in. I am not an esthetician but I have used Cosmelan 1 (mask) and Cosmelan 2 maintenance treatment. Thought you might be interested in my experience. There are apparently a lot of variations in formulas floating around. Since the product is made in Spain, there is a formula sold in Europe, and a different formula sold in the US (or so I have been told). There was originally a yellowish formula, then a dark brown, and now a light brown. There used to be pretty severe redness and peeling from C1 mask but with the one of the formulas now floating around there is no redness or peeling at all. It has been 3 weeks since I did the C1 mask (left it on for 8 hours) and C2 twice a day, and so far I would say the results are okay but definitely not great. I would say I have about a 40% reduction in a prominent age spot on my cheek. A friend who used the older formula had phenomenal results and was the one who recommended Cosmelan to me. Her facialist who performed her treatment about a year ago told her that she doesn't offer Cosmelan anymore since the formula changed and her clients were no longer getting good enough results to warrant the cost. Some sellers on ebay state that they have the "good" formula that peels. It seems to me to be sort of worrisome that a company like Mesoestetic has such a lack of uniformity in its product line. There is also another product that they make called DermAmelan. Not sure if maybe that is like the "old" Cosmelan. It's all very confusing and sort of disturbing that you don't really know what kind of treatment effects or results you will get. Anyone else have any experiences with the "new" non-peeling version? I just did the Cosmelan 1... Im still in the mask!!! I had it done by a Dr. in Santa Monica, quite popular with the celebrities and a big name in TV. He said that he can still get the old one and that is what he used on me today. I will tell you how it goes if you are interested. I researched for months. and met with 4 doctors. Reading posts that talked about the "old" and the "new" and the "pros" and the "cons." I just had to go for it! I was getting dizzy with all the information. Anyway- he said that it would work for me, I hope he is being honest.

I am Day 2 and I am RED!! I look like kind of funny, or at least feel this way! Anyway, like the Dr. said , I am starting to peel. Especially around my mouth and nose. It's not horrible, but still very noticeable. I use the K cream and sunblock daily.

Yesterday, (the day after) I woke up RED and hot. I was kind of scared, so I took some ibuprofen to calm down my fever-like sensations. It worked for the redness and any inflammation from the redness. I did the same thing again today and it worked.

I am waiting to start the C2 until tommorrow. I figure one more day wont hurt. I know that others have said this is where they really peel. I guess I am kind of dreading this part. But, I was told that this C2 is an integral part of the procedure!!!!

I will keep a post...if anyone is reading.

I am on Day 3 and the redness have gone down dramatically, BUT the peeling has REALLY begun. Not scary- just itchy!

I also started using the Cosmelan 2 creme in thin amounts. I started last night (Day 2) and this morning I seem to be fine. I will start today with appyling it twice a day for the next 2 weeks as required.

Ok, that's it! I can't tell much behind the peeling!!!! So, if its working- it's too soon to tell.

I am on Day 11

Flakiness is still present..really have to moisturize!!

Peeled for almost a week. First 2-3 days were the worst. Used c2 2x a day everyday. But put a little extra on spots hours after 2nd application.

Skins looks great!!! Spots are very minimal. Moustache has faded dramatically. I still have some pigmentation from old pimples, but I think they soon will go away.

Post IPL pigmentation spots are almost gone too. I put extra c2 on those at times. They peeled off quicker once I did this!!!

Pros in all areas except one!

My lips are so dry and painful at times, I could barely talk or eat on certain days. I had to put neosporin on them- it worked for burning! But they look very unattractive and dry. One day my lips were so red ... one of my colleagues complimented me on wearing lipstick. hahahahha!! I said what lipstick??? My lips are on fire!!!

Otherwise, if anyones reading and is thinking about cosmelan...do it!! As long as your doctor has old cosmelan and is a good reputable doctor!!!!

Hi. I live in LA and would LOVE to get in touch with your dr. I am a long term Cosmelan user and an Amalan user before that. The new brown version of the formula does NOT work at all. I have been so depressed because no amount of chemical peel, laser treatment, dr. prescribed bleaching creams, or high end products claiming to fade spots has ever worked for me. Cosmelan was my magic answer. I have been so depressed with the new formula. Who is your doctor? Please help...

oh! to anonymous... if you have darker asisn skin, its fine!!! my doctor has done all skin types. i saw asian patients that had nice results, as well as an african-american woman with super dark skin and horrible IPL scarring...she looks way better now... you'll see!!

I had fantastic results when I used the "old" Cosmelan formula approximately 1-1/2 years ago. I recently did the mask again with the new formula and the results are not nearly as good.
Needless to say - I am really disappointed with the new formula. I would also be interested in finding out how I can get my hands on the 'old' formula as well. Thanks so much.

I too did the new formula Cosmelan mask for the 1st time. Initially, my whole face lightened up, red acne scars lightened somewhat and post IPL lightened up a little. On the 4th week, even with continued use of Cosmelan# 2, 2 to 3x a day, results are diminishing and pigment is starting to show. I just got the old cosmelan 2 formula today. I live in Orange County. Apparently, there are doctors who still can order them. Some of the newer users (doctors) carry the new formula, which did not work. I will keep you posted with # 2 Cosmelan.

I hope I hear more from all of you in this forum. I would like to know your experience with the old cosmelan # 2. I will let you know how the old cosmelan# 2 works for me. The new version, Cosmelan 1 or 2 results diminished over time and quickly for me. I heard the old formula is better, so I purchased the old #2 from a doctor in Orange County today. I am told to apply 1x a day for the next 3 days. If no irritation, use 2x day. The consistency is less thick and darker brown than the new version. I found the doctor by just going online in my area and calling to find out what formula they had. It was not too hard to locate. Okay, so I had the NEW formula mask done and the NEW # 2 on my face for the last 4 weeks now. I hoping that the old #2 n will lighten up my pigments dramatically! I hope this is not all confusing. Sorry!

Of course! Phone# 949-727-3800 in Irvine, CA. It's a legitimate doctor's office. If you live further, I am sure you can find doctors out there who still carry them if they have been using Cosmelan for years. The newer doctors are given the newer version, I am told.

I need a favor. What is the consistency and color of the old cosmelan? Is it sort of a tad darker brown and goes on easier compared to the new one? When I put in on, it comes out darker unlike the new one, which is thick and makes me look like a ghost. Also, how long before I see some lightening? Will I peel badly? I have olive skin, but I noticed after using it last night that it is much stronger than the new formula. My face is slightly pink in some areas and dryer. I would appreciate anyone who can share their experience. I hope it works!

well #1 was a odd brownish color, and #2 a light-tan with yellow after tones (that you can see on a towel if wiped off) color that blended in with my light olive skin. its texture is creamy and a little goes a long way. in fact if one puts too much, one gets really red and dry, especially around the mouth and nose...

i talked to my doctor last week and he said that the old cosmelan is the only one that works! im proof of it!!! nothing has ever worked for my melasma and hyperpigmentation!!!!!

i never wear make up anymore. #2 makes your skin look fresh and bright. since i am now only using it 1x a day, my skin is not peeeling, my lips are a little chapped, and my sking tone is even. i kinda miss the redness alittle, because i looked sunkissed all the time and people were saying i looked great!!!

now i use alittle bronzer on my cheeks..thats it!!!!

The old cosmelan# 2 I got has no yellow tint. It's brown, a little goes a long way and blends in smoothly and easily to my olive complexion. I don't see any yellow on it though. I did get it from a reputable doctor yesterday who said it is the old one. Is it the same? Or I think you said that the yellow is seen on the towel only right? Sorry, I need to make sure. I am planning to do the mask if it does not do the trick. I initially did the new formula mask a month ago amd it did not work.

Gosh, I wish i had found this website/forum before today. I just had my Cosmelan 1 mask treatment at the dermo's office this afternoon (Cedar's Sinai Medical Group) and I'm almost 100% sure that it is the new formula since this doctor's office has only been offering this treatment for the past 2 months. I have hyper-pigmentation on my upper lip and eyebrow areas and was really hoping this treatment would work but from what everyone has written about the new formula, I am now preparing myself to be mightily disappointed. I'll try and post back in a few days if I do or do not experience any positive results.

If I decide to try and get the old Cosmelan 2 formula, does anyone know if I have to pay the doctor that orders that a consultation fee or the full service fee for the Cosmelan 1 and 2 application again? That would be prohibitively expensive since I'll have to spend another $800. Thanks for any additional info.

I am an aesthetician and have been performing Cosmelan for a few years now. I am completely disappointed in the new Cosmelan. The older version was very effective. I have called the company (Mesoestetic) several times concerned with the new versions lack of efficacy. They still state that it works as well, it just takes longer... I don't know about that. Either way it is still the only treatment for melasma specifically. Hydroquinones, IPL & other traditional treatments for pigment simply don't work for melasma. I WANT THE OLD COSMELAN BACK

Sorry to report that other than two teeny tiny spots on my upper lip area that lightened, there is no other difference. The Cosmelan2 doesn't seem to do anything at all. Don't waste your money on this new formula.

if you would like to know the contact information for a doctor in santa monica, los angeles that uses old cosmelan, please provide your email address and we can discuss. he doesnt need forum for publicity, in fact, he prefers not to have his name advertised. he does well among the celebrities and charges a bit more, but then again it works!! but, you have to get it done by him and in santa monica. he will not sell it and send it to you. it should be done under doctor care.

if you just want to buy via mail and do at home independently, i have a direct email to dr. krulig, who uses amelan....its the same thing. he is is south america and all he needs is a picture for diagnosis and he WILL send it to you. but my doctor will NOT!!!

by the way, the number JAY provided in the above post for a doctor in orange county is a good doctor. i went to him, but decided to go to mine because it was closer. they both use old cosmelan. i also have a name in the universal city area that uses old one. she was way to far for me, but i keep her contact just in case my doctor runs out. apparently, its hard to get. it must ordered from spain.

I own a spa and I completely agree about the new formulation. I was a huge fan of the old and I am really upset that they changed the formulation. If anyone can tell me or give information on how to order the old formulation for my customers. The last few clients we did cosmelan on was the new formulation and they are unhappy...and I agree. I want to order the old then give them a big discount if they do another treatment with the old verson because I feel bad! I feel so let down by Mesoestetic. When I called them to report this they just fumble around and say the new formulation is the same results but with less side effects...this is SO NOT TRUE!!! I did the new formulation for myself so I could compare (I had done the old formulation about 1.5 yrs ago). The new is NOT the same and I want to order the old!!! Please help if anyone knows how to get it. Do we need to find someone in Europe who can order it and ship it over? What is the deal????
